How to apply for AMCA QC Fellowship |

This document describes the on-line application process for AMCA QC Fellowship.
Account required
Apply on-line
- In order to open the on-line applications you need to log into the system.
- The log-in button is located in the upper right-hand corner of this page.
- On the page that opens (your Personal Site) click on
Applications and Forms/Apply to open the page listing the currently available applications. Click on the title to open the application for AMCA QC Fellowship. IMPORTANT: Do not apply for the membership in AMCA QC.
- If you do not remember your password and you installed your secret question you can recover it on this page:

- If you do not remember your password, request it by sending an e-mail to
password.request@amcaqc.org You will receive an answer within FOUR working days.
DESCRIPTION OF THE ON-LINE APPLICATION
- The application consists of a series of forms described below. Detailed instructions are posted with each form.
- Application is accessible for on-line editing until the deadline date (midnight, Eastern Standard Time = Montreal time).
- Applications are normally opened for four weeks - from March - June (check the announcement for exact date of a particular year).
Personal Record
- The first part of the application deals with your personal data. These data are an essential component of your application, and you should provide all details as requested in the forms.
- Note that once filled out, your personal information is available for any future application on this system.
01. Applications status (Active, Cancelled; this is the form where an applicant can cancel the application) 02. Personal information 03. Photo (150 x 150 pixels, jpg format) 04. Address & telephone 05. Language proficiency 06. Education (from high school on) 07. Interests (One entry only – summarizing your short & long term interests in medicine and research) 08. Experience (include all clinical courses you completed or are currently attending – as well as other medical and extracurricular experience of interest) 09. Publications 10. Awards (list academic awards and any other that you consider as important recognitions) 11. Professional meetings attended (medical or scientific)
Referees
12. Two Referees are required (your responsibility to arrange in advance) 13. (Hidden form) This application will automatically send an e-mail to Referees indicated in Form requesting the letter of reference and providing instructions on how to fill out online the letter of reference.

14. Describe why would this fellowship be important for your development upon return to your home institution.
General
15. Uploaded files: Create and upload a Word file with details of the proposed clerkship as submitted to the McGill Visiting Electives Program (these are important info for synchronization of two applications)
Final
16. Confirmation of the completion of application. In order for the system to register your application, you must click on "Restart".
A brief note on entering data in the forms
- Text prepared in MS Word and copy/pasted in the forms needs to be cleaned from hidden Word codes incompatible with the HTML editor. To remove the code, you can either
- copy/paste in Notepad and then to the forms (preferred way)
- use the filter available in the Editor of this application (" Clean HTML content ").
- The data are saved as soon as you click on "Insert" (for first time entries) or on "Update".
- In areas where multiple entries are allowed, there will be a button "Add New". Once finished with entries, use "Continue". If there are no entries, use "Cancel".
